Randy Fagin, M.D.
Chief Administrative Officer, Texas Institute for Robotic Surgery
Renowned robotic surgeon Randy Fagin, M.D., serves as the chief administrative officer for the Texas Institute for Robotic Surgery. Dr. Fagin is one of the most experienced robotic surgeons in the world and a prominent robotic Thought Leader in the world today. He has lectured and performed thousands of da Vinci surgeries on five continents and has been a faculty member at robotic surgery conferences across the globe. Patients have traveled from 32 states and 12 countries to receive care from Dr. Fagin, and he has performed live surgeries for international meetings for nearly a decade.
Dr. Fagin has authored many book chapters and articles for medical journals on the topic of robotic surgery and serves as a consulting editor for numerous journals. His experience and diverse contributions to robotic surgery have made him a frequently used medical resource for legal experts and the media. Dr. Fagin is the Senior Medical Advisor for Training with Intuitive Surgical and has developed programs for operating-room efficiency, standardization and waste reduction that have been implemented in hospitals across the globe.
Dr. Fagin received his medical degree from SUNY at Buffalo School of Medicine and Biomedical Sciences, and completed his residency in urology at SUNY Health Science Center in Syracuse. He is a board-certified urological surgeon.
Thomas Payne, M.D.
Medical Director
Dr. Payne is both nationally and internationally known as a thought leader in benign robotic assisted gynecologic surgery. He is honored to serve as medical director of Texas Institute for Robotic Surgery. He has authored both book chapters and several journal articles in the field of gynecologic surgery. He serves as faculty for conferences around the globe and has enjoyed the privilege of educating hundreds of his fellow surgeons in the field of robotic surgery. His primary passion is to teach, assist and perform complex surgery for the female patient.
Dr. Payne received his medical degree from the University of Oklahoma College of Medicine, and completed his residency in obstetrics and gynecology at the University of Oklahoma Health Sciences Center. He is a board certified obstetrician-gynecologist.
Charles Kaczmarek, R.N., M.B.A
da Vinci Service Line Officer
Charles received his Bachelor of Science in Nursing from the University of Mary Hardin
Baylor in 2006 and a Master of Business Administration from Texas State University in
2011. His experience with the daVinci robot began in 2007 and in 2008 he became the
daVinci coordinator for the robotic service line at North Austin Medical Center. Due to
his extensive experience in supporting the surgeons, team, and growth of new specialties
along the daVinci service line Charles was asked to join the Texas Institute for Robotic
Surgery in 2011 as the da Vinci Service Line Officer overseeing the efficiency, quality,
productivity, and data collection across all robotic systems and specialties. Charles is
a highly valuable member of our team and serves as a great resource for our Institute,
our surgeons, our team, and the hundreds of healthcare professionals who come to our
educational programs every year.
Madolyn Musick, Administrative Assistant
Madolyn Musick joined St. David’s North Austin Medical Center in 2006, and became a part of the Texas Institute for Robotic Surgery in 2011. Madolyn works closely with course attendees, managing everything from course scheduling and hotel reservations to private transportation and catered dining. It is no wonder that in 2010, Madolyn received the St. David’s North Austin Medical Center “Caregiver of the Year” award, reflecting her personal dedication and passion for her work.

Angie Pack, Data Collection Coordinator
Angie Pack received her bachelor’s degree in health information management from Texas State University and has been on staff at St. David’s North Austin Medical Center since 2009. She joined the Texas Institute for Robotic Surgery in 2011. Angie’s true gift in data organization and management make her an important asset to process implementation,continuous improvement, and educational-program development at the Texas Institute for Robotic Surgery and the Robotic Institute University.
